She had some, soda and some paper towels and bathroom tissue on the bottom rack of the shopping cart, but she made no effort to put the items up on the conveyor belt for the cashier to ring them up. She also did not tell the cashier to bring her little hand-held scanner around to scan the items without having to unload them.

The end result of the transaction was that she did not pay for her items that were on the bottom rack of the shopping cart.  When she joined me at the door, ready to go out to the car I asked her if she had remembered to pay for the items on the bottom rack, and she said that she had. Not quite sure how to confront her or the situation, but I do know that if I ever take her shopping again that I will be joining her at the check out counter and unloading the items on the bottom rack.
